There is no public bank of actual, verbatim NCLEX questions — the exam is computer-adaptive and item-banked, and NCSBN does not release used questions. What you can get are practice questions written to the same NCLEX-RN test plan, covering the same content categories and question formats, which is the closest legitimate substitute.

No source, free or paid, has actual past NCLEX questions, because the exam does not work that way. The NCLEX-RN is a computer adaptive test drawn from a large, continuously updated item bank, and NCSBN treats every question as reusable across future candidates. Once a question is exposed publicly, it is retired from the bank, which is exactly why none circulate legitimately.
What you will find under searches for "actual NCLEX questions" are practice questions written by test-prep companies and nursing educators to match the NCLEX-RN test plan — the same content categories, the same style of client-need scenario, the same answer formats including select-all-that-apply and drag-and-drop. They are not leaked exam content. They are built to feel like it.

The NCLEX-RN test plan, published by NCSBN, breaks the exam into client needs categories: safe and effective care environment, health promotion and maintenance, psychosocial integrity, and physiological integrity, each with defined percentage ranges. Legitimate practice question sets are written against this same framework, which is why good ones feel structurally similar to the real exam even without being the real exam.
Question format matters as much as content. Since the exam moved to include Next Generation NCLEX (NGN) item types — extended multiple response, matrix, bowtie, and cloze — practice that only uses classic four-option multiple choice is training an incomplete skill. Are NCLEX practice questions from prep companies accurate to the real exam?
The better ones are accurate in structure and difficulty because they are written against the same NCSBN test plan, but none are word-for-word matches to real exam content. Check that a source has updated its questions to include Next Generation NCLEX formats, since that is the clearest sign it reflects the current exam rather than the pre-2023 version.
Why does the NCLEX not release old questions like some other exams do?
Because it is a large, continuously replenished item bank rather than a fixed-form test with a handful of retired versions, NCSBN needs to keep questions usable across many future test administrations. Releasing them would shrink the bank and undermine the adaptive testing model that adjusts difficulty to each candidate in real time.
How many practice questions should I answer before sitting the NCLEX?
There is no fixed number that guarantees readiness, since the exam adapts to your performance rather than testing a fixed question count. A more useful marker than a total count is consistent performance across all client needs categories in recent practice, not a rolling raw score from early in your prep.
