Short answer

No public question bank contains actual, currently-used NCLEX RN questions, because the NCSBN retires and rotates its item pool under strict security agreements. What you can get are practice questions written to the same test plan, using the same item types and difficulty calibration as the real exam. The closer a bank matches NCSBN's format, the more useful it is as preparation.

Answering it directly

You cannot legally access actual, currently administered NCLEX RN questions outside the exam itself. Every candidate signs a confidentiality agreement at the test centre, and the NCSBN treats its live item bank as protected intellectual property. Sites or forums claiming to sell "leaked" or "real" NCLEX questions are either recycled retired items presented as current, or fabricated content with no relationship to what NCSBN actually writes.

What does exist, and what is worth your time, is practice content built to the same specifications as the real exam: the current NCLEX-RN Test Plan, the same six client-needs categories, and the same mix of item types, including multiple response, drag-and-drop ordering, and case study formats introduced with Next Generation NCLEX. A well-built bank will not give you memorised answers, but it will train the reasoning pattern the exam is actually testing.

Why the answer is what it is

The NCLEX uses computerised adaptive testing, which means no two candidates sit the same exam. Each question is selected in real time based on your performance on the previous one, drawn from a large item pool calibrated for difficulty. There is no fixed "actual paper" to leak in the way a fixed-form exam might have one. Even if someone reconstructed a handful of questions from memory after sitting the test, that set would represent a tiny, non-representative fraction of the pool, and reusing it would not prepare you for the adaptive logic that decides what you see next.

NCSBN also retires and replaces items continuously as part of routine exam maintenance, so a question a candidate sat in one testing window may not exist in the pool by the next. Exceptions and edge cases

The one exception worth knowing is the NCLEX-RN Practice Exam sold directly by Pearson VUE on behalf of NCSBN. It contains previously used, retired items and is the only product with a genuine claim to having appeared on a live exam at some point. It is limited in scope, useful mainly as a format check rather than a full study tool, and should not be mistaken for a preview of your specific exam.

Be cautious of any resource, paid or free, that markets itself using phrases like "real NCLEX questions 2026" or "leaked exam bank." Beyond the ethical and academic integrity issue of test-security violations, these sets are frequently outdated, inaccurate, or written for older exam formats that no longer reflect Next Generation NCLEX item types. Using them can actively mislead your sense of readiness.

What to do next

Choose a question bank that states clearly which test plan version it follows and includes Next Generation NCLEX item types alongside traditional multiple choice. Run timed sets under exam-like conditions, then review every question you got wrong and every one you got right for the wrong reason, since adaptive testing rewards consistent clinical reasoning rather than pattern recognition.
