What people usually mean by real NCLEX questions is either retired items released by NCSBN in official practice products, or practice questions written to match the current Next Generation NCLEX item types and test plan.

No source, free or paid, legitimately sells you questions currently live on the NCLEX. NCSBN treats the item bank as secure intellectual property, and candidates who sign the exam's non-disclosure agreement and later share questions online are violating it, which can trigger an investigation and invalidate their results. When a resource claims to have real NCLEX questions from this year's exam, treat that as a red flag rather than a selling point.
What does exist, and what is worth your time, falls into two categories: officially retired items that NCSBN itself has released through products like the NCLEX-RN or NCLEX-PN test plan documents and NCSBN-published practice tools, and independently written practice questions built to mirror the current item types, including the six Next Generation NCLEX formats such as extended multiple response, drag and drop, and bowtie items.
